PHARMACY UPDATE
New prescription drug cards will be mailed to Wabash members in December
from EHO. Your new EHO member ID will be four zeros followed by the last
five numbers of your social security number. The new cards will be
effective January 1, 2008. Remember to show your pharmacist this new card
so that their records can be updated as necessary.

ANNUAL PT BENEFIT Effective January 1, 2008, a new methodology for
calculating Physical Therapy Benefits will be implemented. Whereas the
value of the Benefit does not change, new software makes it necessary to change
the wording. The new annual benefit description follows:
Active Employee, Active Sick Leave, Active Cobra, Retired 60/30
~ Physical
Therapy - Calendar Year Benefit 100% to $2,000
75% $2,000 - $4,000
50% over $4,000 (no limit)
~ Rotator Cuff and / or total knee
replacement - Per case / incident
100% to $4,000
50% over $4,000 (no limit)
Furloughed, dismissed, Sick Leave Expired or Resigned Employees paying
their own dues, Disqualified, Retiree under 65 with less than 30 years of
service
~ Physical Therapy - Calendar Year Benefit
80% to $2,000
75% $2,000 - $4,000
50% over $4,000 (no limit)
~ Rotator Cuff and / or total knee
replacement - Per case / incident
80% to $4,000
50% over $4,000 (no limit)
